Melasma and post-inflammatory hyperpigmentation (PIH) are often confused because both cause patches of darker skin. But they have different drivers, and telling them apart matters, because they are managed differently.

What melasma is

Melasma tends to appear as larger, symmetrical patches, often across the cheeks, forehead or upper lip. It is influenced by a combination of factors including hormones, sunlight and heat, which is why it can be persistent and prone to flaring. It is more common in women and in richer skin tones.

What PIH is

PIH follows inflammation or injury, such as acne, a skin reaction or an aggressive procedure. It appears where that inflammation occurred, so it usually maps to previous spots or irritation rather than forming symmetrical patches.

Why the distinction matters

Melasma can be aggravated by heat and by overly aggressive treatment, so it calls for a particularly measured approach. PIH is often more responsive once the underlying inflammation is controlled. Treating one as if it were the other risks poor results or a flare, which is why accurate assessment comes first.
